Your needs will be met with first-rate skill and artistry also with gentleness and care for your comfort. Dr. Timchula has been in practice for over 20 years. Our mission is to provide the highest quality dental
care in an environment that is warm, caring and humanistic to those who share
our values of honesty, trust, integrity and excellence. Because of these values, we strive to lower volume in
order to enhance excellence. As a
restorative practice we hope to go beyond the scope of general practice to
improve self-confidence and self-image through health and beautiful smiles. We share a strong commitment to the personal and

flexibility, security and humor. We have a commitment to leading the field with enthusiasm, and current technology. We warmly share all we know so that those we serve may receive excellence in dental care. Please find enclosed our health history form, fill this
out at your own convenience and bring with you to your appointment. INSURANCEBring all of your dental insurance information in with
you. We will place all of your information on our computer system and print a
completed form for you to mail in for each service. Our office does not accept assignment of benefits which means
that for each visit you will pay Dr. Timchula directly and your insurance will
reimburse you at your home address. We are happy to cooperate in any way we can to help you
receive maximum benefits from your insurance policy. We will assist you in completing all forms.
If you encounter special difficulties, please do not hesitate to ask for
help and advice. We believe in quality care.
We use the best materials, state-of-the-art equipment and the finest
personnel and laboratory services available.
With the growing concern over mercury and health, it is important that
you know we do not place silver mercury fillings. The alternative filling materials include gold, porcelain, or
the new tooth colored composite restorations. Some of our special

for selecting us for your dental care. SPECIAL CONCERNS · Please refrain from wearing perfumes and colognes. We try our best to maintain a chemical free office. · Do not take any supplemental Vitamin C 24 hours prior to your appointment if you are to receive any anesthetic. The Vitamin C will make it difficult for the anesthetic to take effect. We will gladly provide Vitamin C after the appointment to help restore feeling in your mouth. · Be sure to eat before your appointment. We recommend a high protein meal. · Before your first visit it is always helpful to have as much information on your previous dental work as possible. You may even want to call your last dentist to request that a copy of your records be sent to us. · Remember to take your premedication! Premedication is required if you fit into any of the following categories; Ø Prosthetic cardiac valves, including bioprosthetic and homograft valves Ø Previous bacterial endocarditis, even in the absence of heart disease Ø Surgically constructed systemic-pulmonary shunts Ø Most congenital cardiac malformations Ø Rheumatic and other acquired valvular dysfunction, even after valve surgery Ø Hypertrophic cardiomyopathy Ø Mitral valve prolapse with valvular regurgitation Ø If you have any hip, knee, or other artificial joints or prostheses please contact your orthopedist to find out if you will need to take any premedication.
